Pudgy Walsh ordered Tony's death

As David Chase has stated - it's all there. All the evidence and foreshadowing is right under our noses:

1. Pudgy approved non-douglas fir grade joists. Hugh built the houses and Tony as silent partner leaned on inspectors to certify houses up to code, allowing them to make huge profits on the scam for years.

2. The scheme collapses (quite literally) when the roof caves in on Cousin Brian, his wife and child resulting in instant death for all three.

3. Now everyone knows Cousin Brian had powerful friends in high places. Politicians, judges, police commissioners and in the criminal underworld, you name it.. they're furious and out for blood.

4. Pudgy needs to flee but decides to first take care of loose ends. He needs to silence anyone who can link him to this - Tony and Carmella at the diner. Hugh will no doubt be next.

I understand if you're still somewhat sceptical but the final piece brings it all together:

1. Tony sits at a table made of WOOD, possibly even PINE. What's on the walls? More WOOD. Who else is linked to PINE and fatal incidents? That's right, Pudgy Walsh.

Very poetic and fitting. After all the trouble with the New York mob, Tony's death comes at the hand of a heavy set Irishman.
